Sweepstakes Mail-In Entry Instructions

Thank you for your interest in our Sweepstakes.

(b) Mail-In Entry: To enter the Sweepstakes by mail, please print the following information on a 3”x5” card: the words ‘Midnight Velvet’s “Enhance Your Style” Sweepstakes with Marcellas Reynolds,’ your first and last name, date of birth, address (no P.O. boxes accepted), city, state, zip code, telephone number, email address, and the words “I agree to the Official Rules”. Then mail the completed 3”x5” card in a stamped #10 business envelope to: Midnight Velvet Marcellas Reynolds’ Enhances Your Style Sweepstakes, 1112 7th Avenue, Monroe, WI 53566. All mail-in entries must be postmarked by 11:59 pm Central Time on the last day of each entry period (see below). An answer to the question is not required for mail-in entries.

PLEASE NOTE: By entering the sweepstakes via the mail-in entry method, the entrant will not receive future email communications from Sponsor with the exception of email communications relating directly to the administration of this sweepstakes (e.g., winner’s notice) unless the entrant has consented through other means to receiving such communications or has an established business relationship with Sponsor, which permits such communications.

All entries must be received (online entries) or postmarked (mail-in entries) by 11:59 pm CT on the following Mondays: Aug. 29, 2016 (Entry Period #1); Sept. 12, 2016 (Entry Period #2); Sept. 26, 2016 (Entry Period #3); Oct. 10, 2016 (Entry Period #4); Oct. 24, 2016 (Entry Period #5); Nov. 7, 2016 (Entry Period #6); Nov. 21, 2016 (Entry Period #7); AND Dec. 5, 2016 (Entry Period #8). Sponsor is not responsible for lost, late, illegible, corrupted, or misdirected entries.

    I am in the process of moving from my present address, I would like all my billing mail and catalogs form you be mail to my PO Box 24795 Brooklyn, NY 11202.
